Job Description

An exciting opportunity has become available with The CHAOS Group to join the support team as a Team Leader in our new step-down facility in St Erme near Truro. Based on location, this team will be caring for and supporting up to 14 clients who are coming out of the psychiatric wards within the county. The patients are being stepped down as they have a rehab/social need rather than a medical need but are not ready to return home. As Team Leader you will support the Registered Manager and Deputy Manager in organising and
taking on new hospital discharges, completing assessments, overseeing day-to-day operations, managing medication and working closely with the wider team, supporting them to flourish in their roles. You will also be expected to be part of the on-call service for the service which may require availability on occasional weekends/bank holidays.

Key Responsibilities

  • Provide day-to-day support to residents in a 14-bed residential setting.
  • Supporting the Service Manager and Deputy Manager with the day to day running of the service
  • Organising shifts so the team knows what they are doing on a day-to-day basis
  • Working with management, support staff, and clients to understand the needs of those within the service
  • Working with individuals to identify goals and create a robust plan from admission to discharge
  • Working across the sector to provide wraparound support for those within the service to aid recovery
  • Facilitating group activities and 1:1 support sessions with individuals within the service

About You: 

The successful candidate will have the following qualities:

Qualifications:

  • NVQ Level 3 in Health and Social Care or equivalent or willing to work towards this
  • Full clean driving license and own transport
  • Experience working in a health and social care team setting, specifically experience supporting clients with mental health support needs
  • Be able to evidence a good understanding of working in a fast-paced and what can be a challenging environment
  • Good understanding of Mental Health and be familiar with legislation and guidance in place to support the client group
  • Great communication skills, empathy, and a willingness to learn

Skills Required:

  • Excellent organisational and time-management skills
  • Strong problem-solving abilities and the capacity to handle challenging situations
  • Empathy and the ability to build trusting relationships with residents
  • Competence in using basic computer applications for documentation and
    communication
  • Ability to maintain confidentiality and adhere to ethical guidelines
  • Previous medication handling experience

Experience Required:

  • Minimum of 2 years of experience in a mental health support or related role
  • Experience in an accommodation-based service setting, particularly in hospital
    discharge, rehabilitation, and re-ablement, is preferred.

About the Service

The service is based in the village of St Erme, on the outskirts of Truro. Phase One will consist of a building in which up to 14 clients can be supported. This will involve developing plans by working closely with individuals to identify what skills they need to learn and what links they need to make to be able to live independently within their community. These could include social activity, joining a gym or a walking group, volunteering or gaining paid employment, learning practical skills such as cooking or doing their own laundry or just forming positive habits around self-care and nutrition. All of this will be led by the individual on their journey towards recovery.
