Task Force is a front-line 14 day hospital discharge service commissioned by the NHS. This hybrid role is delivering hands on care to patients within their own homes and being responsible for undertaking assessments. You need to be excellent at record keeping and information sharing, have exemplary communication skills and a minimum of three years’ delivery experience in Health and Social Care.
Task work with a wide range of professionals and the ideal candidate would be forward thinking, adaptable and a team player.

Essential Skills:NVQ Level 3 in Health and Social Care or Management
2 Years of Care Experience is preferred
Full, clean driving license and own transport
