An opportunity to join our fast-paced, exciting team as a field supervisor. Task Force is a front-line 14-day hospital discharge service commissioned by the NHS. This hybrid role is delivering hands-on care to patients within their own homes and being responsible for undertaking assessments. You need to be excellent at record keeping and information sharing, have exemplary communication skills and a minimum of three years’ delivery experience in Health and Social Care.
Task Force work with a wide range of professionals and the ideal candidate would be forward-thinking, adaptable and a team player. A rewarding role in a supportive working environment with good career progression for the right candidates as part of a dynamic, knowledgeable team. Please note, driving is essential for this role.
