About the role

As an Outreach Support Worker, you will:
Provide 1:1 support in the community and at home
Support individuals to access education, employment, and social activities
Help develop daily living skills (budgeting, cooking, travel training, routines)
Promote confidence, independence, and emotional wellbeing
Build positive, trusting relationships
Maintain accurate records and communicate effectively with the wider team
This is an active, rewarding role where no two days are the same.

A typical day

No two days are the same — and that’s what makes this role so rewarding.
Your day might start by supporting a young adult to build their morning routine and travel independently to college or work. Later, you could be helping someone develop life skills such as budgeting, meal planning, or cooking a new recipe at home.
In the afternoon, you might support someone to access the community — whether that’s attending a gym session, going to a social group, volunteering, or simply building confidence in public spaces. Throughout the day, you’ll encourage independence, promote emotional wellbeing, and provide consistent, positive support.
You’ll keep clear records, communicate with the wider team where needed, and always ensure support is person-centred and aligned with individual goals.

Key Responsibilities

Provide person-centred 1:1 support in the community and within individuals’ homes
Support individuals to access education, employment, training, and social activities
Assist with developing daily living skills, including budgeting, cooking, travel training, and routines
Promote independence, confidence, and emotional wellbeing
Support individuals to achieve their personal goals and outcomes
Build positive, trusting, and professional relationships
Encourage community participation and reduce social isolation
Follow individual support plans and risk assessments
Maintain accurate and timely records of support provided
Communicate effectively with families, professionals, and the wider team
Report safeguarding concerns or changes in needs promptly
Work in line with company policies, procedures, and regulatory standards
Participate in training, supervision, and ongoing professional development
