The Role
We are looking for an organised, proactive, and compassionate Care Coordinator to join our growing team. The Care Coordinator plays a key role in ensuring the smooth day-to-day running of our care services by coordinating care packages, supporting care staff, and ensuring clients receive consistent, high-quality support.

Coordinate and schedule care visits to ensure safe, consistent, and effective service delivery
Match carers to clients to promote continuity and strong working relationships
Act as a key point of contact for clients, families, and care staff
Respond to changes in care needs, rota gaps, and unplanned issues in a calm and professional manner
Support recruitment, onboarding, and induction of new care staff
Maintain accurate records, rotas, and care documentation
Work closely with management to ensure compliance with policies, procedures, and regulatory requirements
Support quality monitoring and continuous improvement across the service
Communicate effectively with professionals, commissioners, and external partners where required
Contribute to a positive, supportive team culture
