About the role

Glorylilly Care is looking for an enthusiastic and motivated Team Leader to support clients in their own homes and lead our care team. You’ll ensure high-quality, person-centred care while mentoring staff and contributing to care planning and reviews.

A typical day

As a Community Care Team Leader, you will provide day-to-day guidance and support to field staff during office hours, liaise with health and social care professionals, and maintain regular communication with clients and their families. You will also carry out spot checks and quality monitoring to ensure safe, effective, and person-centred care is consistently delivered.

Key Responsibilities

Lead, support, and mentor care staff.
Ensure delivery of safe, high-quality care in line with individual care plans.
Communicate effectively with clients, families, and professionals.
Contribute to care reviews and client outcome planning.
Attend care calls as needed and participate in monthly on-call rota.
Promote client independence, dignity, and wellbeing.
