Pay: £13.50 – £15.00 per hour
(Enhanced rates of £14–£15 after 6 pm and additional weekend enhancements)

Your Role: As a Reablement Worker, you’ll play a vital part in supporting individuals as they transition from hospital back to their homes. This role requires compassion, excellent communication skills, and a genuine desire to make a positive impact every day.

Key Responsibilities of a Reablement Healthcare Assistant
Support individuals to achieve goals outlined in their reablement care plan.
Assist with personal care, including washing, dressing, grooming, and toileting, while encouraging independence.
Help people regain mobility and confidence in moving around safely.
Support daily living activities such as meal preparation, shopping, medication prompts, and household tasks.
Monitor and report changes in a person’s physical or mental wellbeing.
Work closely with healthcare professionals, social workers, therapists, and family members.
Maintain accurate records of progress and care provided.
Promote dignity, choice, and person-centred care at all times.
Follow safeguarding, health and safety, and infection-control procedures.
Provide encouragement, motivation, and emotional support to help individuals meet their rehabilitation goals.
