Salary: £26,500
Working Hours: 37 hours per week
Contract Type: Fixed term contract 10 months – Maternity Cover
Base: West Building, Bodmin Highways, Castle Canyke Road, Bodmin, PL31 1DZ
About the role: Are you an organised, detail-oriented individual with a passion for supporting people and processes? We're looking for a proactive Scheduler Discharge Coordinator to join our dynamic STEPs team. In this key role, you'll work collaboratively with colleagues from different teams, disciplines and partner organisations in order to provide a seamless service to individuals/families/carers.

Key Responsibilities

Role Purpose: 
Schedule and carry out planned visits to service users to produce initial reablement / support plans, including initial risks assessments, moving and handling risk assessment to minimise the areas of risk to service user, staff, service providers and the general public or refer to other community services, within the first three days and may include initially the delivery of hands-on care, ensuring information is shared with relevant parties.
